Discharge Medication Review Service to continue

Dr Mathew Smith, Dr Delyth James, Dr Louise Hughes and Dr Karen Hodson with colleagues from the Universities of South Wales and Bradford have recently completed an evaluation of the discharge medication Review service in Wales for the Welsh Government.As a result of the findings in this evaluation the Welsh Government Health Minister, Mark Drakeford, this week approved the continuation of the scheme. This has been reported to the national media and in the Pharmaceutical Journal.
