Person-Centred Care
The Division of Population Medicine hosts a team of health services researchers and academic clinicians working in Person-Centred Healthcare.
This team has expertise in:
- establishing the best ways to embed person centred healthcare and shared decision making approaches into routine healthcare settings
- self-management support
- health literacy
- communication in consultations and across patient pathways
- ensuring under-represented groups are supported to participate in research and healthcare decisions
- development, evaluation, and implementation of patient reported outcome measures and patient reported experiences measures
Our work focuses on improving health and care delivery, experiences, and outcomes, as defined by patients and service users.
We draw on the approaches of patient empowerment and co-production to engage patients, carers and members of the public in their health and healthcare.
We research ways to support patients to develop the knowledge and skills to more effectively manage and make informed decisions about their own health and healthcare.
We translate our research into new models of care delivery, teaching and training for medical students and health care professionals.
We also work with key stakeholders to embed these approaches into routine clinical care.
