HC3141 - Foundations in Community and Practice Nursing

The module has been designed to develop the skills and theoretical knowledge of the community and practice nurses to provide future high quality care.

Community and practice nurses are in a pivotal position, both opportunistically and through planned intervention, to influence the health status of the estimated 98% of the population who work within a community and primary care setting. Community and practice nurses are taking a leading role in achieving the reforms required for the modernisation of the NHS.

Both the significance and variety of roles within community and practice nursing has increased as primary care services expand and chronic disease management shifts from secondary care into the community. Community and practice nurses therefore require additional knowledge and skills to be effective practitioners within the primary health care team and this module has been developed to equip you with a clear understanding of the evolving community and practice nurse role.

This module will focus on the assessment, planning, implementation and evaluation of care given to individuals, families and groups within the context of community and practice.
