Promoting Health and Managing Complex Care - 30 credits (NR3176)

Chronic health conditions need to be managed effectively within the community in order to avoid inappropriate admission to hospital and this requires holistic assessment to ensure prompt identification of problems. Both Practice and District Nurses provide care in a range of settings and are well placed to deliver health education, health promotion and chronic disease management. The role of primary care is seen as central and will involve working with patients and other agencies in the management of chronic disease.

The overall aim of this module is to develop the knowledge and skills of District and Practice nurses in promoting health, and to prevent and manage disease within the primary care setting. The ability to prescribe safely and competently from the Community Practitioner formulary will also be addressed.
