- Working with individuals with severe and enduring mental health problems in acute crisis.
- Providing specialist support in the individual’s home.
- Working in Crisis Recovery, Crisis House
- Care plan review meetings
- Early discharge assessment
- Planning & implementation of home treatment care plans
- Liaison with aligned CMHT’s
- Visiting local hostels, non NHS care providers & user lead services
- MHA 2007 assessments
- Management of daily shift, i.e. screening referrals, allocation of work delegation to all disciplines within the team
- Medication issues, concordance & administration
