Statistics show that one in four of us are living with one or more long term conditions. These people are observed to be the biggest consumers of health care. The second annual ‘Tackling Long Term Conditions conference’ aimed to answer common questions about this area, with a programme of speakers discussing the future of health care and detailing practical examples which are effecting real change.

The focus of Judith’s talk was on the importance of providing systematic and evidence based care in the primary care environment, which took into account of the expert patient and reduces unnecessary hospital admissions. Judith discussed the content of her book, “Managing Long-term Conditions and Chronic Illness in Primary Care”, identifying the key elements of practice that clinicians need to consider in order to provide effective care. To find out more about Judith’s book please see our  Managing Long-term Conditions and Chronic Illness in Primary Care page.
