Pre-Assessment

The Pre-Assessment Questionnaire allows you and the assessor to prepare for the assessment.  Once you have an appointment for a DSA Needs Assessment it is important to complete the pre-assessment questionnaire as it provides the assessor with information about your disability and support you may already have in place.  For further information about the assessment procedure please look at the Assessment Process.
